1. Contact your insurance company right away to make necessary changes so that you can be seen at your earliest convenience. Be sure to specify that you want to be a patient of Dr. Anthony C. Zwaan and mention our new location: 19 Hampton Road, Suite One in Exeter to avoid confusion. We are contracted with most major insurances.
2. Contact us to register. You can print out this two-sided form (PDF) to help you get started. Be sure to complete both sides, sign and date all sections.
3. Arrange for transfer of records from Exeter Family Medicine. You can print out this form (PDF). Be sure to complete the entire form and check off the shaded areas and forward it to Exeter Family Medicine Associates.
