If you wish to join our practice:
1. Check with your insurance company.
If you need to select a Primary Care Provider (some HMO plans require this), specify
Anthony C. Zwaan, M.D. at his new location: 19 Hampton Road, Suite One, Exeter, NH 03833.
Please be sure that this change is in effect prior to your first visit with Dr. Zwaan to
avoid problems.
As of May, 2005 we accept Aetna, Anthem, most Blue Cross & Blue Shields, CIGNA, Harvard Pilgrim, HCVM/First Health, Great-West/One Health, Medicare, PHCS, and United Healthcare.
Patients with non-contracted insurances or self-paying patients will be asked to pay at the time of service. We will gladly provide documentation you can submit to your insurance.
2. Register
A registration form can be printed from this website. Be sure to complete both sides
prior to your appointment, sign and date all sections, and either mail it in or bring
it on your first visit. Contact the office to register and schedule an appointment.
We encourage you to schedule an appointment to discuss any medical concerns, or a physical
to get (re)acquainted and (re)established.

3. Records
Arrange for transfer of records from your prior primary care physician, as well as any additional records that may be helpful (especially immunization records for children).
Allow 3-4 weeks for transfer and review of records. If possible, allow for transfer of records before your first appointment. A record release can be printed from the website. Be sure to complete the entire form, check off the shaded areas, sign it, and mail or FAX it to the intended person/entity.
